This fixed the leak with a bit of help. I took off the old one, and as other reviews stated needed to use the existing screw.Also the surface on the disposal underneath this seal was corroded due to the prolonged leak and calcium deposits. I had to clean off surface and apply silicone to seal the imperfections for this to work. Ultimately I will need to replace the disposal. This did buy me some time before I do a remodel.

This combo flange/gasket fits perfectly. My configuration has a straight pvc pipe from the disposal tailpiece to the main pipe. The only caveat, as other reviewers have noted, is that the screw provided doesn't fit certain models. I ordered the Insinkerator 1491 screw here on Amazon, which is a perfect fit for Insinkerator 175-60351. This assembly makes the disposal quieter than before. Great to find this item and save old disposal.

this is a direst replacement and fit perfectly. There is one screw to secure this discharge port to the garbage disposal and that did not fit, seemed like the wrong thread pitch or something. I had the screw from the old part so I used that and it was not an issue. I would buy this again.
